Therapy Service Packages

I will start taking appointments in January of 2023.

Please note that my service packages are designed to serve long term adult clients, ages 18 and up. Research shows that long term therapy has greater chance of long term success. If you are looking for brief therapy or two-week stabilizations, this practice will not be the right fit for you. Long term in this context refers to anywhere from six months to multiple years.

Initial Session

Please note that the first session is an evaluation, takes 60-120 minutes, and is billed separately. The first session includes intake documents, the informed consent process, a biopsychosocial history, any necessary assessments, evaluation and treatment planning, and the creation of your private, secure therapy and training website with individualized learning content. During that initial session, we will spend some time getting to know each other, and we will discuss your goals and what to expect from therapy. I will answer any questions you have about the therapy process. If you have questions prior to the session, please fill out this form. The informed consent agreement must be completed prior to this first session, and a payment method must be stored before an appointment will be scheduled.

HIPAA-secure Email Communications

Email coming to you from this domain is HIPAA-compliant on my end only. For this reason, I use a service called Hushmail for HIPAA- secure emails containing any content related to your therapy. Any email you receive from me via Hushmail will prompt you to click on a secure link that opens in a protected web page. The content of the message displays on that protected web page and does not display in the email. When you complete the initial contact form, you will be given the Hushmail email address from which you will receive my emails. For cyber-security reasons, it is not listed on this Site. All contact forms for this business are HIPAA-compliant.

One Hour of HIPAA-secure Text Messaging

With Tier 2, you add the option to communicate with your clinician by text messaging for more than just setting up or confirming appointments. This Tier offers HIPAA-secure text messaging totaling up to one hour throughout the week from 10am-10pm seven days/week. Text time includes messaging in both directions. Text time is estimated based on an average reading speed of 130 words per minute. Texting will be primarily asynchronous, meaning that you may not get an immediate response. You will receive an initial reply within four hours or less and can continue to send messages between 10am-10pm, seven days/week. Additional text time is available in prepaid increments of $30 per any used portion of 30 minutes. Fees for additional text messaging may be charged to your payment method either daily or weekly. There is no additional charge for the app itself but you will need to sign up via invitation. You will be sent an invite to install the HIPAA-secure app once you have signed the informed consent agreement. You must sign the informed consent agreement prior to your first virtual counseling appointment. Your first appointment will be set up through HIPAA-secure email messaging.

No Suprises Act and the Good Faith Estimate

As of January 1, 2022, under the No Surprises Act, all healthcare providers were asked to provide transparency regarding fees for healthcare services. The spirit of this legislation is to protect consumers from receiving unanticipated medical bills. Although my practice is self-pay only (aka private pay or direct pay), it is my policy to provide a good faith estimate (GFE) to all clients whether or not they intend to submit to claims to insurance for possible reimbursement. This GFE will be provided both orally and in writing.

The No Surprises Act requires the inclusion of a diagnosis for all GFEs. This is a source of contention for many clinicians, including me, for two reasons: 1) We question the ethics of diagnosing a client before meeting with them, and 2) one of the reasons we choose private practice is to mitigate the stigma of locking clients into diagnoses as required by insurance companies and the medical model. It is, however, required of us to included a diagnosis on the GFE, and to ask if you intend to submit a claim for insurance reimbursement. Note that per my policies, I do not deal with insurance companies. I do not provide superbills or correspond with insurance companies in any way. Note that it is unlikely that you will be reimbursed without a superbill, so please take this into consideration when choosing a therapy practice.

For initial sessions, the diagnostic code I will use on your GFE is called a Z code. A Z code is a descriptor that indicates the need for treatment services. After we meet for your initial session, I will update your diagnosis if applicable and provide a new GFE that reflects the update. In compliance with this new law, I will provide you with a new GFE any time there are changes to your diagnosis, frequency of sessions, or cost of sessions.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. Save it in a secure location.

Notes about Services

Counseling Service Caveats

Fees for all counseling service packages are charged to your payment method in advance on a weekly basis. If there is no current, unexpired payment method on file, scheduled services will be automatically canceled. Clients are responsible for ensuring the payment method is current and is updated. If the payment method on file indicates there are insufficient funds, scheduled services will be canceled and access to the private client website and learning content will be suspended.

Counseling services are available only to clients located in Florida at the time of service. I am currently licenses in the state of Florida only. By law, any client for whom I provide counseling services must be located in the state of Florida at the time of service, regardless of whether that service takes place in person or online. If you are an out-of-state resident but are physically located in Florida, I can counseling services for you while you are in the state. If you are a Florida resident but currently working, traveling, or vacationing out of the state, I cannot provide counseling services for you. If you reside in another state and would like me to look into licensure in your state, please complete this form.
